DS-4085: Should you use this form?

Form DS-4085 should only be used to add visa pages to a currently valid U.S. passport. Otherwise please click here.

Please note that although the form indicates that persons residing abroad may not use it by mail to the indicated U.S. address, this restriction does not apply to resident of Canada. Complete the form and follow other instructions as indicated.

Checklist

Important: Applications which are incomplete, or which make use of the wrong form, or which are directed to the wrong office will be returned to the applicant for correction. You may apply by mail or in person at your nearest U.S. Embassy or Consulate.

In your application, make sure to enclose the following:

• Completed and signed Application for Additional Visa Pages Form DS-4085, which is also available at your nearest U.S. Embassy or Consulate.
• Your valid U.S. Passport.
• Non-refundable fee of $82.

Please note that the fee is payable by U.S. cash, Canadian dollar equivalent or credit card. Bank or money orders must be in U.S. dollars only and made payable to the Department of State. Personal checks are not accepted.
